She is survived by: her husband James Perkins; her sons, Jeff Perkins (Lora) of Clearfield and Ronnie Perkins (Melanie) of Clearfield; her grandchildren, Jason Perkins (Kara) of Clearfield, Josh Perkins (Mendy) of Clearfield, Jeremy Perkins (Krista) of Clearfield, Abigail Bradley (Jordan) of Clearfield and Gabe Perkins of Clearfield; and her siblings, Donald Evans (Mary Lou) of Galion, Gerald Evans of Lucas, Ohio and Mary Evans of Columbus, Ohio. She is also survived by six great grandchildren.
