He is survived by : his sons, Cliff Chapman of Louisville, KY, Jay Chapman of Morgantown, KY and Lance Chapman of Morgantown, KY; his sisters, Lynda Herrington of Bowling Green, KY and Nancy Cox of Colorado; his grandchildren, Kellen Chapman of Richmond, KY, John Daniel Chapman of Bowling Green, KY, Rebecca Chapman of Central City, KY and Meredith Chapman of Russellville, KY; and his caregivers, Patti Knight, I of Morgantown, KY and Linda Woodard of Morgantown, KY. He is also survived by several nieces and nephews.
