He is survived by: his wife Donna Crowe Church; his daughters, Misty Church, Holly Church Mayfield, Katie Church (Jonathan Bloniarz) and Cassie Boehman (Logan); his grandchildren, Andrew, Chase, Michael, Maya, Cameron, Emma Jo, Jamison, Collin, Vincent, Jaxston, Hudson, Gracelyn, Raylan and Stetson; his great grandchildren, Brayleigh and Lyla Lou; and his sisters, Rhonda McCain, Susan Floyd (Jackie) and Karen Arnold. He is also survived by nieces, nephews and cousins.
