He was predeceased by: his parents, Chuck Moore and Rose Moore. He is survived by: his wife Terri Moore of Radcliff, KY; his children, Charles Leroy Moore of Boston, KY and Stacy Ross (Rob) of Vine Grove, KY; his siblings, Sherry Cornelis of South Bend, IN and Lonnie Moore; and his grandchild Bobby of Linsay.

In lieu of flowers the family request that donations be made to any favorite charity of choice.
