He was predeceased by: his parents, James "Jim" Puckett and Maxie Cook Puckett; and his brothers, George Puckett, Don Puckett and Charles Puckett.
He is survived by: his wife Mary Puckett; and his siblings, Dorothy Conley, Francis Puckett, Shirley Brown, Cathy Lain (Jimmy), Lester Puckett (Bonnie), James Puckett (Patricia), Phillip Puckett (Jacqueline), Johnny Puckett, Margaret Puckett Workman, Genevieve Puckett and Mignon Puckett.
In lieu of flowers, family has requested contributions made to the Salyersville International Pentecostal Church of Christ, at P.O. Box 255, Salyersville, Kentucky 41465.
