He was predeceased by: his parents, Lacy Collinsworth and Lizie Powers Collinsworth; his wife Roncie Howard Collinsworth; and his siblings, Paul B. Collinsworth, Hazel Lemaster, Helen Lemaster, Carla Sue Adams and Garnett Lemaster.
He is survived by: his siblings, William Samuel Collinsworth (Evelyn) of Salyersville, Fred Collinsworth (Brenda) of Salyersville, Lloyd Collinsworth (Barbara) of Adrian, Kathryn Phillips (Roger) of Winchester and Janie Chism of Winchester. He is also survived by nephews, and nieces.
Visitation was held on Tuesday, October 14th 2025 from 6:00 PM to 9:00 PM. A funeral service was held on Wednesday, October 15th 2025 at 11:00 AM at the Magoffin County Funeral Home Chapel (900 Parkway Dr, Salyersville, KY 41465).
