Collinsworth (Evelyn), Fred Collinsworth (Brenda) of Salyersville, Lloyd Collinsworth (Barbara) of Adrian, Kathryn Phillips (Roger) of Winchester and Janie Chism of Winchester; his grandchildren, Julia Collinsworth, Laci Jane Lemaster, Blake Collinsworth, Landon Collinsworth, William Collinsworth, Stephen Lemaster (Jessica) and Bobby Lemaster; his great grandchildren, Carter Lemaster and Corbin Lemaster; his sisters, Helen Lemaster, Hazel Lemaster, Garnett Lemaster and Carla Sue Adams; and his brothers-in-law, Tommy Chism, Wayne Lemaster, Wiley B. Lemaster and Jesse Lee Adams. He is also survived by nieces, nephews family, and friends.
