He was predeceased by: his parents, Clyde Maxie and Pearl Delevine Carter Church; and his son Chadvic Church. He is survived by: his sons, Bradley Church (Victoria) and Daniel Church (Anna); his brothers, Charles Church (Sondra) and Steven Church (Cindy); his grandchildren, Nicholas, Cheyenne, Victoria, Logan, Brittany Ann and Chadvic Lee; his great grandchildren, Ava, Kolton, Alexis, Alexander, Anthony James, Trinity, Jacob Ryan, Arianna and Mary Scarlet; and his great great grandchildren, Myles and Keziah. He is also survived by several nieces and nephews.
