He was predeceased by: his parents, Blanton Slack and Ruth Lukemeier Slack; and his sisters, Wanda West and Carolyn Jean McMenimon. He is survived by: his wife Dorothy; his sons, Daniel Slack and David Michael Slack (Elizabeth); and his grandchildren, Vivian and Frankie. He is also survived by several nieces and nephews; and a host of relatives and friends.
