He was predeceased by: his parents, John William and Ann White Slayback; and his grandparents, John and Lavinia Slayback. He is survived by: his pet Trixie; his wife Julie Lonkard Slayback; his children, Damon Slayback (Melanie) and Curtis Slayback; his grandson Garrett Slayback; and his siblings, Thomas Slayback (Betty) and Sheryl Maynard (John). He is also survived by many nieces, nephews, cousins, and close friends.
