He is survived by: his wife Amy Cureington Slade; his children, Clifton Slade, Clayton Slade, Lauren Martin and Jonathan Barlow; his siblings, Cathy Jo "Slade" Lewis, Carol "Slade" Redden and Johnny Slade (Sherry); his stepmother Sherry McGee; and his grandchildren, Everleigh Martin and Garrett Martin. He is also survived by several nieces and nephews, great nieces and great nephews.
