He is survived by: his wife Sherrie Cook; his children, Paxton Cook (Morgan) and Katie Cook; his mother Naomi Cook; and his siblings, Keith Cook (Sherri), Mark Cook (Brenda) and Dianne Lewis (David). He is also survived by 8 nephews and 1 niece.
