She was predeceased by: her husband Major Harold E. Forman; her son Gregory E. Forman; her parents, Meta Jaschke and Henry Jaschke; and her sister Melba Duke.
She is survived by: her daughters, Lisa Broussard (Larry) of Carencro, LA and Marcie Moore of Lafayette, LA; her granddaughter Paris H. Mouton (Mark) of Lafayette, LA; her siblings, Doris Medlin of Santa Rosa, CA, Judy Self of Victoria, TX and Del Jaschke of Victoria, TX; and her daughter-in-law Sara Mueller of Baton Rouge, LA. She is also survived by numerous nieces and nephews.
