He was predeceased by: his parents, Horace Jacob and Beverly Jacob. He is survived by: his wife Melody Betancourt Jacob; his child Jerry Victor Jacob, Jr. (Sandy Crist); and his brother Ronald Jacob, Sr. (Joyce Jacob). He is also survived by three grandchildren, two great grandchildren, 7 nieces and nephews.

In lieu flowers memorial contributions may be sent to the American Diabetes Association.
