He was predeceased by: his parents, Herman S. Diaz, Sr. and Irene Hebert Diaz; and his siblings, Herman S. Diaz, Sr., Ronald R. Diaz, Robert B. Diaz and Susan M. Diaz.
He is survived by: his wife Margaret Slattery Diaz; his children, Robin Ann Diaz of Chalmette, LA, Dennis Gerard Diaz, Jr. of Picayune, MS and Michelle Maria Diaz of Slidell, LA.; his sisters, Brenda Diaz of Pearl River, LA and Carol Ann Lichenstein of Houma, LA.; his friends, Paula and Lee Seward; Pauline Gioe (Tina). He is also survived by his wife of 30 years, his children, in-laws.
