She was predeceased by: her husband Bronislaw Forys. She is survived by: her son Michael Forys; her daughter-in-law Susan Forys of Chicopee; her daughter Margaret Nummy; her son-in-law Alan Nummy of Southampton, MA.; and her grandchildren, Benjamin Nummy (Joanna) of Winnetka, Katherine Plourde (James) of Brentwood, Brendan Forys and Sean Forys (Nicole) of Springfield.
