He is survived by: his pet Dakota; his parents, Christal and David; his brothers, Bryce and Lars; and his grandparents, Hannelore Munson, Karl and Jan Bjork. He is also survived by many cousins, aunts, uncles and close friends.
In lieu of flowers and gifts, the family has requested contributions be directed to the Chase Bjork Foundation, a non-profit dedicated to supporting those affected by mental illness.
