He was predeceased by: his parent Joaquim B.; and his sibling Marie Lanzillo. He is survived by: his spouse Joyce Anne Pereira (Scully); his parent Marie Pereira (Azevedo); and his children, John Pereira (Lycia) of East Taunton, Jeffrey Pereira (Kelly) of Norton, Joanne McLeod (Greg) of Plymouth and Jennifer Hall of Virginia. He is also survived by 13 grandchildren and 9 great-grandchildren.

In lieu of flowers, donations in Jack's memory may be sent to the Legacy House, 9505 SW 110th St., Ocala, FL 34481.
