He was predeceased by: his parents, Muriel V. Jasiak (Porter) and Charles T. Jasiak of Holden. He is survived by: his wife Ann-Marie Jasiak (Strand); his daughters, Tammy (Kevin Allen), Audra (Scott McNamara) and Jessica (James Bourque); his great grandchild Peyton Allen-Cross; and his pet Reagan. He is also survived by his wife, lifelong friends.

In lieu of flowers, the family kindly requests donations to The Baypath Humane Society in Hopkinton and the First Congregational Church of Boylston.
