She was predeceased by: her husband Michael J. Forman. She is survived by: her sons, Bruce Forman of Delray Beach, FL, Mark Forman of Hull, MA and Eric Forman of Coconut Creek, FL.; her daughters-in-law, Carol Forman and Robyn Forman; and her grandsons, Matthew Forman (Alyssa Buell), Brad and Scott Hickox. She is also survived by many nieces, nephews, and extended family.
