She was predeceased by: her parent Mary Katherine Repak (Michalczyk); and her aunt Elizabeth Michalczyk. She is survived by: her siblings, John Repak, Thaddeus Repak and Michael V. Michalczyk, Sr.; and her siblings' children, Victoria LeBlanc, Nicole LeBlanc, Derek Michalczyk, Patrick Lupa, Joseph Lupa, Kathleen LeBlanc, Michael V. Michalczyk, Jr., Doreen Lupa, Renee Napoletano, Jacob Rueli and Adam Michalczyk. She is also survived by extended family members and dear friends.
Visiting hours are on Thursday from 5:00-7:00pm and In lieu of flowers, donations may be made in Theresa's memory to Baystate Children's Hospital 759 Chestnut Street Springfield, MA 01199.
