Warner V. Slack Obituary

It is with great sadness that we announce the death of Warner V. Slack (Newton, Massachusetts), who passed away on June 23, 2018, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He is survived by: his wife Carolyn; his children, Alison, Jennifer and Charlie; and his grandson Sean Peters.

In lieu of flowers, the family suggests donations be made to the Warner Slack Scholarship for Clinical Informatics, 1330 Beacon Street, Suite 400, Brookline, MA 02446.
