He is survived by: his wife Julie; his children, Trista Lachapelle (Mike) of Providence, Joseph Fowler of Lawrence and Matthew Fowler of Providence; his grandchildren, Colin Anne and Kylie Anne; his siblings, Donna Phaneuf (Chris) of North Andover, Joseph of Andover and Beverly Varney (Charlie) of Exeter; and also, Jeffrey, Christopher and Drew.
In lieu of flowers, donations can be made in Jack's name to the Lawrence General Hospital in Lawrence MA.
