He is survived by: his parents, Carolyn Sr. (Whalon) and Frates Sr. of Taunton; his children, Jared Azevedo of James, III, Matthew Azevedo of Taunton and Kristen Azevedo of Taunton; his siblings, Kimberly Frates of Taunton, Anthony Azevedo of Taunton, Keith Azevedo and Austin Azevedo; and also Danielle Azevedo (Roy) of Taunton.
