He is survived by: his wife Julia Hazard (Lusk); his children, Nichole Lusk of Worcester, Kimberly Dion (Alex) of Webster, Andrea Lusk of Worcester, Jessica Lusk of Worcester, Caitlin Hazard-Gonzalez (Rene) of Lake Worth, FL., Melinda Raymond (Jason) of Worcester, Jacob Kelley of New Bedford, Lewis James Hazard, III of Worcester, Dylan Howley of New London, CT. and Jorel Townsend of Auburn; and his siblings, Bruce Hazard of North Brookfield, Robert Bell of Worcester, Betty Ann Bell of Worcester and Joe Walley of Worcester. He is also survived by several nieces, nephews, extended family members and friends.
