He was predeceased by: his parents, Raymond Chouinard and Leona Chouinard of Spencer, MA; and his brother Donald Chouinard. He is survived by: his siblings, Frances Chouinard Lubans of Worcester, Carol Chouinard of Belchertown, Richard Chouinard (Veronica) of Northborough, Roger Chouinard (Carol) of Provo, UT, Bruce Chouinard (Cindy) of Ware and Gary Chouinard (Virginia) of Tucson, AZ; his siblings' children, Ryan, Beau Chouinard of Provo, UT, Brian Chouinard (Cheryl) of Worcester, Matthew Chouinard (Vivian) of Orlando, FL, Deborah Blodgett Hoag of Worcester, Lynn Chouinard Shepherd of Barre and Beth Chouinard Weisenberg of Northborough; his friend Anthony; and his niece Deb. He is also survived by cousins, great nephews and grandnieces.
