He was predeceased by: his parents, Sgt. Ismael Diaz and Hilda Aponte Diaz; and his brother Jose Diaz. He is survived by: his wife Rose; his children, Anthony (Brittany), Tenley and Stephen; and his siblings, Jennie Guilbaud and Chris Flester.

In lieu of flowers, contributions can be made to St Jude Children's Research Hospital.
