He was predeceased by: his wife Edith R. Horowitz; his granddaughter Rebecca Jenna Horowitz; and his parents, Sarah Horowitz and Bernard Horowitz. He is survived by: his children, Dr. Mindy Horowitz (Mark Mellman), David Horowitz (Nancy), Judith Horowitz (Joshua Ottenberg) and Dr. Ruth Horowitz (Carl Shanholtz); his sister Marcia Resnick (Michael); his grandchildren, Yedeedya Mellman (Betsy), Aryeh Mellman (Talya), Mira Mellman, Jacob Horowitz (Jessica Rodriguez), Samuel Ottenberg, Juliet Ottenberg, Adina Shanholtz (Victoria Kealiher) and Elise Shanholtz; and his great grandchild Genesis Rose Horowitz.
