She was predeceased by: her parent Walter Layman; and her brothers, Walter Mumma, Jr. and Gary Mumma. She is survived by: her parent Betty Jane Mumma (Suffecool); her husband John Polk; her daughters, Chase Polk and Shelby Polk; her niece Jessie Raines; and her nephew Gary Ingham.
