He was predeceased by: his parents, James Johnson and Sally Johnson. He is survived by: his wife Cheryl Ann Johnson; his daughter Bre-Anne Stephanie Caudill (Justen Lee, Sr.); his sister Linda Lee Kennedy (Thomas Michael); and his sibling's children, Thomas Michael Kennedy, Jr. and Lindsey Lee.
