He was predeceased by: his great grandchild Brennan. He is survived by: his wife Dorothy Kolakowski; his children, Maura Taylor (Mike), Bethany DeLude and Cara Kolakowski-Jacobs (Chris); his parent Polish; and his grandchildren, Elizabeth, Michael, Coleman, Monica, Keegan, Rory and Harper. He is also survived by his wife, three loving daughters.
