He was predeceased by: his parents, Dr. Charles J. Blazek, Jr. and Florence Blazek; and his brother Gary. He is survived by: his wife Monique Blazek; his sibling's children, Michael P Colegrove (Sarah) and Kathleen Colegrove-Calvey (Joe); his children, Shane, Sam, Jesse and Putter; and also Madison.
Due to COVID-19 all services are private.
