He was predeceased by: his parents, Arthur Fortin and Elaine Fortin (Gagne). He is survived by: his wife Caroline Fortin (Loukides) of Frenchville; his son David Kostyk (Shelly Dawn Sherren) of Colorado; his stepson Scott Fongemie of Lewiston; his granddaughters, Lauren of Colorado and Emily of Rumford; his great grandchild Roy of Colorado; and his brothers, Patrick Fortin (Louisselle) of Madawaska and Roger Fortin (Lorraine) of Connecticut. He is also survived by many nieces and nephews.
