He was predeceased by: his parents, Frank Slovak, Sr. and Mary Slovak (Frola); his wife Dora Guerette; his daughter Joan Parker; and his siblings, Frank Slovak, Leda Slovak, Hattie Bryant, Bessey Henry and Mary Groleau.
He is survived by: his children, Paul Slovak of Jay, Thomas Slovak (Lori) of Livermore Falls and Pauline Slovak of Las Vegas. He is also survived by 5 grandchildren and 6 great-grandchildren.
