She was predeceased by: her parents, Amedee Joseph and Irene Maire Dumont LeBlanc; her husband Carleton Jack; and her sisters, Patricia Sargent and Theresa Pyburn.
She is survived by: her daughter Nancy Kiley of Beverly, MA; her stepchildren, Douglas Hayden of Minneapolis, MN, Lesley Hayden of Washington, DC and Heather Hayden Mason of Boston; her siblings, Donald LeBlanc (Peggy) of Wellington, ME and Nancy Wood of Lynn, MA; her grandchildren, Shauna Martin of Salem, MA and Adam Martin of Beverly, MA; and her great grandchildren, Isabella Moore of Salem, MA and Jameson Martin of Beverly, MA. She is also survived by nieces, nephews, and cousins.
