He was predeceased by: his parents, George Appleby and Gloria "Wallace" Appleby; and his brother Terrance Rholoff. He is survived by: his daughters, Denise Bernius and Krista Appleby (Nikki); his wife Kelly; his sister Jean Appleby; and his grandsons, Jason and Alex.

In lieu of flowers his family asks that a donation be made on his behalf to the Michael J. Fox Foundation for Parkinson's Research.
