She was predeceased by: her parents, Paul Fultz and Ethel Fultz (Saylor); her brother Gary Fultz; and her husband Mark Cousins. She is survived by: her daughter Valerie Hackworth (David) of Clarksville, TN; her grandchildren, Kelsea Bernklau (Jesse), Kyler Hackworth and Kaleb Hackworth; her great grandchildren, Mila Bernklau and Ada Bernklau; her siblings, Vivian Lewis of Homer, Jan Russell of Battle Creek and Greg Fultz of Somerset, KY; and also Douglas Arnett. She is also survived by nieces and nephews, her first husband.
