She was predeceased by: her parents, Cecil Baker and Arvilla Baker (Harrington); and her husband Gerald Smith. She is survived by: her children, Donna Wachtel (Shawn), Jerry Smith Jr., Ellen Smith and Steve Smith; her siblings, Phyllis Amick, Judy Bishop, Ella Mae Ash, Jeannette McQuarter, Diane Gladieux and Cecil Baker Jr.; her grandchildren, Derek Browne, Sarah Shannon, Joshua Wachtel, Alexis Budzinski and Sophia Belt; and her great grandchildren, Isabella, Olivia, Aiden, Uriah, Riley and Jace. She is also survived by many nieces and nephews.
The family suggests memorial contributions in Hazel's name be made to the Salvation Army.
