He was loved and cherished by many people including: his child Mary Smith (Scott); his stepchildren, Kathy Burdick and Ron Burdick; his grandchildren, Sam Jr., Grace (Trevor), Jennifer, Anthony, Brian (Theresa) and Maria; his stepgrandchild Bryan; and his wife Theresa.

In lieu of flowers, memorials are appreciated to Seasons Hospice.
