He is survived by: his parents, Leroy Mathews and Vicki Mathews; his wife Jenny Mathews; his children, Dakota Mathews and Rashawna Burtis; and his sisters, Suzy Cormendy (Mike), Shelly Weis (Henry) and Sandi Carpenter (Terry). He is also survived by several nieces and nephews; and 3 great-nieces and nephews.

In lieu of flowers, contributions may be made to the Alzheimer's Association or the American Cancer Society.
