He was predeceased by : his parents, Ellis David and Arlene Rose. He is survived by : his wife Dana Frost; his son Jack David Frost II; his son-in-law Lindsay Frost; his sisters, Pamela Jeronis (Chas), Lillie Reynolds and Ethel Morea (Richard); his friends, Joyce Cummings, Sharon Cummings, Diane, Rick Schultz, Sue, Phil Osborne, Michael and Marn Kremski; and his godson Dustin Hook. He is also survived by many nieces, nephews.
