He was predeceased by : his parents, Paul Cook and Barbara Cook. He is survived by : his children, Tom Cook (Deb), Tim Cook, Rebecka Neff (Chad), Jamie Kuiper, Ken Cook and Diana Cook; his grandchildren, Brenton Kolenda, Breanna, Haley, Sophia Cook, Emmanuella, Elliott, Sawyer Neff, Molly, Micah Dewey, Jacob and Caleb Kuiper; his great grandchildren, Owen Fochtman and Myla White; and his siblings, Steve (Sue), Dan Cook (Carrie) and Barbara Handley (Terry). He is also survived by nieces and nephews.
