He was predeceased by: his parents, Fred Lewis and Lilly Lewis (McDonald); his parents-in-law, Mae (Spradling) and Jesse White; and his siblings, Desta Ament (Lewis), Mack Lewis and David Lewis.
He is survived by: his wife Mary Alice White; his children, Vickie Berens, Jennifer Streur (Mitchell), Donald Jr. Lewis (Lori), Patrick Lewis (Leah), Kyle Lewis (Tammy) and Christopher Lewis (Erin); and his sister Marian Cowell (Alan). He is also survived by several brothers-in-law, sisters-in-law, nieces, and nephews.
