He is survived by: his mother Josephine Evangelista (DeGeorgeo); his wife Vicki Evangelista (Blocher); his children, Jason Douglas Evangelista, Jennifer Dawn Richardson (John) and Heather Marie Evangelista; his siblings, Michael Evangelista and Lori Girard (Dennis); and his grandchildren, Sienna Josephine and Annalise Marie. He is also survived by many nieces and nephews.
Memorial contributions may be given to family for his grandchildren's college fund.
