He is survived by: his parents, Tammi Woodwyk and Tyler Woodwyk; his grandparents, Carla, Robert Woodwyk, Lyle and Jan Fraaza; his aunts and uncles, Tonya Longoria (Eddie), Chad Fraaza, Heidi Ter Haar, Ryan Fraaza (Tamara), Julie Veltema (Brad), Paul Fraaza, Tracy Rios (Marco), Michaela Woodwyk-Reiffer (Riley) and Hannah McGhan (Brent); and his great-grandparent Katherine Woodwyk.

Memorial contributions may be given to the NICU Unit at Helen Devos Children's Hospital, 100 Michigan St. NE, Grand Rapids, MI 49503, Attn: NICU Unit.
