He was predeceased by: his parents, Carl Shaw and Nancy Shaw (Hancock); and his siblings, Ruby Herrick and Orville Serr. He is survived by: his wife Dora Sereseroz; his daughters, Tina Levi (Kevin) and Teresa Holguin (David Jr.); his grandchildren, Quentin Levi (Celene), Camerin Levi (Ashley), Makayla Cinnamon (Cody), Kyler Holguin and David Holguin; and his great grandchild Elias Levi.

In lieu of flowers, memorial contributions can be made to the Shaw family.
