He was predeceased by: his parents, Anna Mallak (Reis) and Walter Mallak; his wife Karen Roth; and his siblings, Sharon Martin, James Mallak, Duane Napora and Butch.

He is survived by: his children, Gary Mallak (Melissa), Susan Fisher (Brian), Angela Lederer and Jamie Mallak (James VanOchten); his brother Thomas Mallak; and his friends, Denny and Wendy Lafond. He is also survived by many nephews and nieces.
