He was predeceased by: his parents, Onufre Bazan and Anastasia Bazan (Bodnar); and his siblings, Pete, Anna, Tilly and Olga.
He is survived by: his wife Jean Bazan; and his daughters, Jennifer Bazan and Stephanie Bazan. He is also survived by several nieces and nephews.
