He is survived by: his children, Dr. Lisa Marie Rothman (Joshua) and Casimir "Cass" Walter Cook II (Deanna); his grandchildren, Jacob Rothman, Jared Rothman, Daphne Cook and Gideon Cook; and his siblings, Zuzanna Piekos, Jozef Kuc and Mary Kuc.
