He was predeceased by: his mother Theresa Lynn Addis (Berry). He is survived by: his father Dale L. Addis of White Cloud; his siblings, Dwayne Addis (Tanya) of Vestaburg and Crista Phillips (Sean) of Vestaburg; and his grandparents, Ray and Sheila Berry of Newaygo. He is also survived by several aunts, uncles, and cousins.
